New Client Info
NEW CLIENT PORTAL

For new patients, prior to our first appointment, I will send you an email to create a log-in to my client portal. There will be new client paperwork to read and sign through this portal.

For existing clients, you can access the portal here.
CONFIDENTIALITY AND PRIVACY POLICY
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.

Exceptions include:
-
Suspected child abuse or dependent adult or elder abuse. I am required by law to report this to the appropriate authorities immediately.
-
If a client is threatening serious bodily harm to another person/s, we must notify the police and inform the intended victim.
-
If a client intends to harm himself or herself, we will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, we will take further measures without their permission that are provided to us by law in order to ensure their safety.

24-HOUR CANCELATION POLICY

Therapy works best when scheduled appointments are regularly kept. If you need to cancel an appointment, it is important that you contact me and let me know as soon as possible about your need to cancel. Alternative appointments will be offered, if available.

If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be responsible for the full out-of-pocket cost for the session.

CRISIS AND URGENT SITUATIONS
We will respond to messages and phone calls as soon as possible. However, we do not provide urgent, crisis, or emergency services. If you are experiencing a life threatening crisis and require immediate assistance, dial 911, or go directly to your nearest emergency room immediately.
